Request for Lab Exam

Reminder, ISP Lab Request form (CHPD 2025-26 ISP Request for Lab Exam Form) must be completed from  the ”CHPD SHARED/FORMS & GUIDES/Dept FORMS/Lab Request Forms” folder at CHPD or via FORMS/GUIDES in the OFFICER AREA of ctownpd.com.

* If you are unsure what needs to be sent to the lab, contact the requesting clerk at the prosecutor’s office, for clarification.

* Request form must include the court date, and names of the individuals associated to each item of evidence submitted, the date of seizure, and the designation of ONE ITEM as the primary probable cause item (one per case).

* Use ”SAVE AS” to save the COMPLETED request form to your Officers Folder with the file name as such, including the respective department case number (EXAMPLE):  CHPD 2025-1234 Lab Request

* Email the COMPLETED and SAVED Request form from your Officers Folder, to cledbetter@ctownpd.com, the remainder of transportation and processing of the request will be taken care of by Property Officer Ledbetter.
