Report (Case, Accident, etc.) copies or Local Record Checks on yourself can be obtained from the Charlestown Police Department, at 701 Main Street, Charlestown, Indiana, between the hours of 8:00 am and 4:00 pm, Monday through Friday. There is a $5.00 charge for all report services.
You can also purchase Indiana Crash (Accident) Reports on-line by using BUYCRASH.COM.
Notice: If you are involved in a motor vehicle accident, you are required by Indiana State Law to submit a completed Indiana Operator's Proof of Insurance/Crash Report (SR-21 form) to the Indiana Bureau of Motor Vehicles, within ten (10) days of the accident. The SR-21 form is available at the Charlestown Police Department, any Indiana State Police Post, any Indiana BMV License Branch, and on-line by CLICKING HERE. Completed SR-21 forms should be mailed to: PFR/Crash Report Section, P.O. Box 7169, Indianapolis, IN 46207 within ten (10) days of traffic accidents to avoid driver license suspension.
Note: Indiana Criminal History Record Checks must be obtained through the Indiana State Police. For more information on obtaining Criminal History Records, CLICK HERE. Criminal History Reports can not be obtained publicly via local Law Enforcement agencies.
